2024-2025 Forms

Aid for Part-Time Study (APTS) Application

Students who are enrolled for less than 6 credits and are eligible for NYS funding can now apply for Aid for Part-Time Study (APTS). Submit this form along with your NYS IT-201 Taxes. Students MUST complete a NYS Payment (TAP) application to be considered for an award.


Asset Verification Form (Student/Parent)

This form provides information on the student and parent(s) assets which is needed to determine financial aid eligibility.


Identity and Statement of Educational Purpose (In Person)

To verify his or her identity, the student must provide the ORIGINAL notarized Identity and Statement of Education Purpose Form, as well as an original valid government-issued ID, such as, but not limited to, a driver’s license, other state-issued ID, or a passport.


Identity and Statement of Educational Purpose (To be signed with a Notary)

If the student is unable to appear in person at SUNY Morrisville's Financial Aid Office to verify his or her identity, the student must provide the ORIGINAL notarized Identity and Statement of Educational Purpose form, as well as a copy of the valid government-issued photo identification (ID) that is acknowledged in the notary statement, such as, but not limited to, a driver's license, other state-issued ID, or a passport. The original form may then be mailed to our office.


Satisfactory Academic Progress - Federal Waiver

This form is used by students who are appealing the loss of their federal financial aid. This form and documentation must be submitted to the Financial Aid Office no later than 30 days after being notified that you are not making Satisfactory Academic Progress (SAP).


Special Conditions Form

This form allows for an explanation of changes in financial circumstance that a student or a family may be experiencing (such as adjustments to income, family size, or explanations of one-time payments, capital gains, etc.) as reported on your FAFSA.


Summer Aid Request Form

This form is to be completed by students who will be taking summer courses and would like financial aid to cover them.


TAP Ineligible Waiver

This is for students who are ineligible for their TAP due to failing to meet satisfactory academic progress (SAP) as noted in the College Student Handbook. Students who did not meet these criteria are eligible to complete this ONE-TIME waiver to get their TAP reinstated. You must provide an explanation that was beyond your control and provide third-party documentation to back up your reasoning. This form and all documentation pertaining to this will be submitted to the Office of the Registrar for review.


TAP C-Waiver

This is for students who are ineligible for their TAP due to failing to meet the GPA requirement portion of satisfactory academic progress (SAP) as noted in the College Student Handbook. Students who did not meet these criteria are eligible to complete this waiver to get their TAP reinstated. You must provide an explanation of the circumstances beyond your control that prevented you from being academically successful as well as third-party documentation to back up your reasoning. This form and supporting documentation will be submitted to the Financial Aid Office for review.
