Financial Aid Forms

Financial Aid
  • Through the Aid for Part-time Study program, New York provides tuition assistance to eligible undergraduate students enrolled on a part-time basis in a program of study at a participating institution in New York State.  The application, along with copies of the most recent NYS Income Tax Returns, must be submitted to the Financial Aid Office at Morrisville State College prior to the start of the semester.

    Aid For Part-Time Study
  • Students enrolled and matriculated at Morrisville State College (MSC) may receive financial aid for those courses taken at another institution if those courses are applicable to their program of study. In order to receive financial aid, federal regulations require that a Consortium Agreement must exist before a home institution can process financial aid for students attending another institution (Host).

    Consortium Agreement
  • This form is for students that are required to have parental information on their 2016-2017 FAFSA form and are selected for income verification. This form is to be completed by the student and parent(s).

    Dependent Verification Worksheet
  • Students may grant Morrisville State College permission to release information concerning their student records to a third party by submitting this completed Consent to Release Student Information Authorization Form.

    Family Educational Rights and Privacy Act (FERPA)
  • The student must appear in person at Morrisville State College’s Financial Aid Office to verify his or her identity by presenting this form and a valid government-issued photo identification (ID), such as, but not limited to, a driver’s license, other state-issued ID, or passport.  The institution will maintain a copy of the student’s photo ID that is annotated with the date it was received and the name of the official at the institution authorized to collect the student’s ID. 

    Identity and Statement of Educational Purpose (To Be Signed at the Institution)
  • If the student is unable to appear in person at Morrisville State College’s Financial Aid Office to verify his or her identity, the student must provide the original notarized Identity and Statement of Educational Purpose, as well as a copy of the valid government-issued photo identification (ID) that is acknowledged in the notary statement, such as but not limited to a driver's license, other state-issued ID, or passport.

  • This form is for students that are not required to have parental information on their FAFSA form and are selected for income verification. This form is to be completed by the student and spouse, if married.

    Independent Verification Worksheet
  • This form allows for explanantion of changes in financial circumstance that a student or a family may be experiencing, such as adjustments to income, family size, or explanations of one-time payments, capital gains, etc. as reported on your FAFSA.

  • This form is to be completed by students who will be taking summer courses and would like financial aid to cover it.

  • This form is for students who are in their 4th semester of receiving TAP and failed to meet the overall 2.0 GPA satisfactory progress.

    C Waiver Form

  • This form is for confirming your selective service registration and is for males between the ages of 18-25. All males must be registered with the selective service in order to receive financial aid.

  • This is for TAP eligible students who failed to meet satisfactory progress as noted in the College Student Handbook. Students who did not meet these criteria are required to complete this ONE TIME waiver to get their TAP Award reinstated.
